Online Grant Sponsorship Donation Request Application

INSTRUCTIONS: Please review the form below in advance for all the information that is required, and have that information ready to be entered. When you are ready to submit your application, fill in the form below, upload your Cover Letter, upload your signed Terms & Conditions pdf document, sign this form, and press the SUBMIT button to submit your form for processing. All fields marked as (*) are required.

NOTE: Completion of this form does not guarantee that the VI Lottery will fulfill this request. Please fax your completed form, along with a cover letter addressed to the Executive Director on your organization’s letterhead to (340) 776-4730, attention DONATION REQUEST APPLICATION.

Required Documentation:

  1. 501(c)3 Non-Profit Certification
  2. Current Certificate of Good Standing (certification must be dated within one (1) year from the date of sponsorship application submission)
  3. Cover Letter (addressed to Mr. Raymond Williams)
  4. Signed VI Lottery Community Enrichment Rules & Regulations Document (found on our website)
