When you are ready to submit your application, fill in the form below, upload your Cover Letter, upload your signed Terms & Conditions pdf document, sign this form, and press the SUBMIT button to submit your form for processing. NOTE: Completion of this form does not guarantee that the VI Lottery will fulfill this request. Please fax your completed form, along with a cover letter addressed to the Executive Director on your organization’s letterhead to (340) 776-4730, attention DONATION REQUEST APPLICATION.

Current Certificate of Good Standing (certification must be dated within one (1) year from the date of sponsorship application submission)
Cover Letter (addressed to Mr. Raymond Williams)
Signed VI Lottery Community Enrichment Rules & Regulations Document (found on our website)
